Key Responsibilities
- Assess the psychosocial status of patients and families related to terminal illness.
- Conduct social evaluations and develop intervention plans based on findings.
- Maintain clinical records and provide information and referral services.
- Act as a liaison between patients, families, and community agencies.
- Participate in interdisciplinary meetings and quality improvement activities.
Qualifications
- Bachelor’s degree in social work accredited by the Council on Social Work Education.
- Minimum of one year’s experience in healthcare; hospice experience preferred.
- Strong verbal and written communication skills.
- Current CPR Certification and valid driver’s license.
