New Beginnings Home Care, Inc. is looking for a motivated, driven Registered Nurse to join our team and make a difference in the community!
Requirements:
- Professional RN with current unrestricted license (minimum of 1 year experience preferred but not required)
- Valid NYS Drivers License
Specifics:
- Hybrid position (4 days in office 1 days remote) 90 days required in office training period at start of employment
- Hours are 7:30am-4 pm, Mon-Fri; Must be flexible when Agency needs arise
- Field work/travel required
- Salary includes weekly gas stipend
- Preferred experience (but not required): working in home care, working with Traumatic Brain Injury and/or Dementia patients, as well as supervisory/management experience
Roles/Responsibilities:
- Case manager/administrative position responsible for overseeing the provision of care to clients receiving home care services between Seneca, Cayuga and Onondaga counties (caseload to include potential for 20+ clients)
- Responsible for ongoing evaluation and assessment of all clients on caseload in order to write and implement a plan of care for home care services
- Responsible for conducting assessments necessary for the start of care, recent hospitalization discharges and every 6 months ongoing to ensure the necessary provision of services
- Responsible for ensuring that home care services are being carried out specific to each client
- Responsible for overseeing/supervising the Personal Care Aides (PCAs) working in client's homes that fall under client caseload to ensure they are following the plan of care. Responsible for educating/training PCAs on their responsibilities within the client's homes, as well as addressing any concerns with PCAs that may arise.
- Responsible for acting as a liaison between our clients, their family members and the PCAs working in their homes
- Responsible for assisting with ongoing agency trainings – teaching PCA Training Class, Orientations and Annual In-Service trainings
- Be available for an On-Call rotation to give guidance to On-Call supervisor regarding any medical concerns that arise during non-business hours (rotation includes 1 - 2 weeks per month based on need)
- Must be able to work independently
Paid Time-off Benefits:
- Paid holidays after 90-days of employment (can be paid for non holiday days of closing like Black Friday, based on performance and at manager discretion)
- After 90-days of employment, 120 hours of PTO. After 1 year, receive 160 hours. Grows each year after
- At hiring receive 16 hours and then 40 hours at 90-days. 56 hours annually thereafter
