Department & Position Overview:

In the role of a Care Coordinator, you will be the linchpin connecting clients, caregivers, and various departments within our organization. Your primary objective is to ensure a seamless customer service experience for our clients, all while making sure they receive the care they require. Every Day You Will:

• Manage an assigned client caseload, ensuring their needs are met.
• Adhere to a revolving monthly client service and compliance standard to maintain our high standards of care.
• Be the direct point of contact for enrolled client members, addressing their questions, inquiries, and concerns.
• Utilize your experience in data input, case review, and building rapport and confidence with clients.
• Multitask efficiently, exhibit excellent written and verbal communication skills, and maintain strong organizational abilities.

Ideal Candidate Will Possess:

• A proven track record of delivering exceptional customer service.
• The ability to maintain poise and patience when interacting with customers.
• Preferred 2 years of prior experience in a similar position.
• A strong motivation to meet and exceed case management targets and metrics.
• Previous experience in the medical field and/or case management is a definite plus.
• Proficiency in both written and verbal communication.
• Familiarity with computer and phone operations.
• Bilingual skills, including Spanish or other languages, are highly desired.
