Position Overview
We are seeking a Geriatric Nurse Practitioner (APRN) with a specialization in geriatrics to join our expanding team. This role is ideal for an experienced provider passionate about elder care and looking to deliver patient-centered, in-home medical services. You will work in close collaboration with our Licensed Clinical Social Workers (LCSWs) and facility staff to support holistic, integrated care.
Responsibilities
- Provide comprehensive, in-person medical care to older adults in their homes, assisted living, and independent living communities
- Perform geriatric assessments, diagnose and manage chronic and acute conditions common to aging populations
- Coordinate care with behavioral health professionals (LCSWs), caregivers, and facility staff
- Order and interpret diagnostic tests and manage medications appropriately
- Maintain accurate and timely documentation in accordance with clinical and regulatory standards
Qualifications
- Required: Active APRN license with a specialization in Geriatrics in Utah
- Required: Experience working with elderly or medically complex patients
- Comfortable working independently in the community (home and facility visits)
- Strong interpersonal, diagnostic, and documentation skills
- Valid driver’s license and reliable transportation
