SUMMARY:
Under indirect supervision, provides rehabilitative and developmental therapy support services for patients in the home setting. Directs patient participation in selected tasks to restore, reinforce, and enhance performance. Facilitates the learning of skills and functions essential for adaptation and productivity, diminishes or corrects pathology, and promotes and maintains health.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Email Monitoring/Timely Response
- Plan, organize, and conduct occupational therapy programs designed to help patients regain physical or mental functioning or adjust to their disabilities.
- Effective Patient Communication
- Request supplies and equipment as needed.
- Practice safety, environmental, and/or infection control methods.
- Perform therapeutic exercise, with and without equipment.
- Complete visit charting and submit within 24 hours of visit time.
- Share new information or concerns with staff in a timely fashion.
- Communicate patient progress/status as relates to the treatment plan.
- Complete all required digital forms for patient continued care and discharge.

EDUCATION AND EXPERIENCE
Bachelor's Degree in Occupational Therapy
Registered member and in good standing with the provincial College of Occupational
Member of the Canadian Association of Occupational Therapists and/or the provincial Society of Occupational Therapists
Minimum six months of experience that is directly related to the duties and responsibilities specified.
Ability to observe, assess, and record symptoms, reactions, and progress.
Ability to monitor and/or maintain quality control standards.
Knowledge of clinical operations and procedures.
Ability to interpret, adapt, and apply guidelines and procedures in the home setting.
Ability to plan, develop, and implement physical therapy programs.
Ability to submit visit documentation within 24 hours of the visit.
