PRN Medical Social Worker

Serving Jefferson County residents, LMSW will assist in the enrollment process for comprehensive medical services within company contracts.



ESSENTIAL DUTIES AND RESPONSIBILITIES  


 Develops or assists with those of the organization to ensure efficient and safe operation of the  unit.  

 Plans and facilitates community outreach and education activities, as appropriate to the position;  may serve as a liaison and representative to community organizations and schools.   Performs various administrative functions, such as monitoring budgets, preparing administrative  and programmatic reports and correspondence; participates in a variety of research projects to  develop service plans and propose changes to program policies and procedures.   Provides casework management of a specialized caseload; performs screening and assessment;  prepares individualized studies for admission, placement, and /or discharge.  

 Provides counseling, therapy, and /or psychotherapy to clients and families as appropriate to the  position; prepares treatment plans, discharge plans, and follow-up care programs; provides  therapeutic crisis intervention and emergency services as required.  

 Coordinates and/or schedules medical treatment activities with medical staff; may participate as a  member of an interdisciplinary team.  

 Documents clients’ activities and progress; writes social and/or medical summaries.   Provides consulting services to other patient care professionals.  

 Maintains current knowledge base on social work issues through conferences, educational  requirements for licensure, and social work literature.  

 Performs miscellaneous job-related duties as assigned.

QUALIFICATIONS

 You must have the ability to gather data, compile information, and prepare reports, have some  knowledge of medical terminology.

 You have to have strong interpersonal and communication skills and the ability to work  effectively with a wide range of constituencies in a diverse community.  

  

EDUCATION and/or EXPERIENCE  

 Baccalaureate or Masters degree in Social Work  

 Current registration and in good standing with provincial College of Social Work   Three to five years experience directly related to the duties and responsibilities specified.   Successful candidate must submit to post-offer pre-employment physical/medical history   Required to submit to annual TB and Rubella screen  

 Must provide record of Hepatitis vaccination or declination  

 Willingness to comply by participation in any training BrightStar provides as required by  provincial Occupational Health & Safety legislation to ensure employee safety  


COMMUNICATION SKILLS  

 Ability to write routine reports and required correspondence as required by BrightStar guidelines.  

 Ability to respond professionally on behalf of BrightStar.  

 Must be able to appropriately respond to and maintain confidential information at all times.

Requires the ability to prioritize and complete work on time, be detail-oriented, exhibit a service  orientation to both staff and clients and be a professional team player with a positive attitude. Relies on  experience and judgment to plan and accomplish goals. A certain degree of creativity and latitude is  required. Must possess the ability to handle confidential information and act as the BrightStar  spokesperson. Must also possess the ability to respond appropriately in an objective manner in all  situations. Excellent listening skills are essential. Requires outstanding organizational skills, problem  solving skills, thoroughness, adaptability to shifting priorities and multi-tasking. Requires exceptional  interpersonal skills and exhibit a service orientation towards others. Must have the ability to demonstrate  solid judgment and discretion. Must also demonstrate a sense of understanding and urgency for priorities.  Will require the ability to appropriately troubleshoot and escalate where necessary, problems/issues to the  appropriate individual.  


TECHNICAL SKILLS 

Requires familiarity with standard concepts, practices, and procedures within a particular field.  


PHYSICAL DEMANDS  

The physical demands described here are representative of those that must be met by an employee to  successfully perform the essential functions of this job.  

 Employee must be able to perform the essential functions of this job, such as physical  requirements including (but not limited to) climbing stairs, the ability to lift/transfer patients and  drive.  

 Successful candidate must submit to post offer, pre-employment physical examination/medical history  check and background check. 

 May be required to submit to annual TB and rubella screening. Ability to safely lift, and physically  manipulate patients.  

 Work is normally performed in a typical interior/office/home environment  

 Moderate physical activity, occasional lifting up to 25lb, standing or walking for more than 4  hours per day  

 Home environment involves possible exposure to potentially dangerous materials and situations  that require following safety precautions and may include use of protective equipment.
