Hospice On-Call Registered Nurse

Join our hospice's after-hours nursing team and become a part of our caring and supportive family.
Overview:
- Work schedule: 7 days on / 7 days off (shift is from 5pm on Friday until Monday 8am)
- Salaried position
- Be available outside of normal business hours to address any patient needs
- Enjoy your free time during the 7 days off
Duties & Responsibilities:
- Conduct patient visits and gather information for the RN Case Manager
- Perform treatments, including wound care, as directed by the RN Case Manager
- Assist in identifying discharge-planning needs
- Report changes in the patient's physical condition or care needs to PCP
- Order medication, DME, and other supplies as needed
- Participate in quality assessment and performance improvement activities
- Ensure seamless transition of patients to hospice care
Job Requirements:
- Current licensure to practice professional nursing in Texas
- Ability to assess patient's needs and direct care accordingly
- Good communication, negotiation, and public relations skills
- Demonstrate autonomy, organization, assertiveness, flexibility, and cooperation
- Valid Driver's License and automobile in good working
- HCHB/Pointcare knowledge preferred, but not required.
