At Emblem Hospice Tucson, we are dedicated to offering life-changing service to our patients. As a Hospice Chaplain, you will have the opportunity to provide essential spiritual care services to patients and their families/caregivers, either directly or through coordination with other spiritual counselors. You will play a vital role in assuring the spiritual well-being of our patients and providing them with comfort and support during their hospice journey.
Job Description Summary:
- Assess the spiritual needs of patients and families/caregivers in a timely manner
- Participate in the development of individualized care plans to meet identified spiritual needs
- Provide direct spiritual care to patients and families/caregivers
- Serve as a liaison and support to community chaplains and spiritual counselors
- Maintain records of spiritual care services utilization for quality assurance
- Attend interdisciplinary group meetings and provide consultation and support on spiritual care
- Recruit community chaplains, spiritual counselors, and volunteers to meet patient needs
- Participate in quality assessment performance improvement teams and activities
- Plan funeral or memorial services for patients as requested
- Supervise spiritual care volunteers assigned to patients/families/caregivers
Position Qualifications:
- Graduate of an accredited seminary or school of theology
- Experience working with death and dying individuals/families/caregivers
- Ability to work as a member of an interdisciplinary team
- Comfort in an interfaith setting
- Ability to accept different lifestyles, cultures, beliefs, and values
- Knowledge of and commitment to hospice philosophy of care
- Must be a licensed driver with a reliable, insured vehicle
- Hospice experience preferred
