Part-Time or Per Diem RN for Hospice Admissions
The Admission Registered Nurse plays a pivotal role in the education process for
patients, families and caregivers. The Hospice Admission Nurse guides patients
and families through the hospice assessment and admission process utilizing
excellent compassionate active listening.
Duties and Responsibilities
·Complete an initial, comprehensive assessment of patient, interview and educate
patient’s family and/or caregivers regarding hospice philosophy and goals.
·Review medical records and communicate with Hospice Medical Director Clinical
Leadership Team to determine Hospice eligibility and appropriateness.
·Develops an initial plan of care that establishes goals based on hospice
diagnosis and incorporates palliative nursing action. Includes the family and
facility staff, if appropriate, in the planning process.
·Completes all admission documentation within expected time frame
·Orders medication and profile the patient with the pharmacy including orders
DME as necessary.
·Other duties as assigned
Qualifications
·Registered nurse with current unencumbered licensure to practice professional
nursing in Arizona.
·Prior Hospice nursing is preferred but not required.
·Demonstrates excellent observation, problem solving, verbal and written
communications and nursing skills per competency checklist.
·Shows ability to organize and prioritize workload independently.
·Maintains a current CPR certification.
·Must be a licensed driver with an automobile that is insured in accordance with
state or organization requirements and is in good working order.
·Self-directed and able to work with minimal supervision.
