Job Summary
- Plan, organize, and direct home care services focused on community health education.
- Collaborate with individuals and families within their homes to meet unique needs.
Duties & Responsibilities
- Patient Care: Develop personalized care plans, implement nursing procedures, and provide education to patients and families.
- Communication: Update physicians on patient conditions and coordinate care plans with community health professionals.
- Additional Duties: Participate in quality assessment programs, supervise home health aides, and ensure necessary equipment availability.
Job Requirements
- Graduated from an approved nursing school and licensed in the state of practice.
- Minimum of two years of experience, preferably in public health or home care.
- Current CPR certification and a reliable vehicle.
- Excellent observation, problem-solving, and communication skills.
- Ability to work independently and prioritize tasks.
