Champlain Valley Senior Community Urgent Hiring: Immediate Opening for a Highly Skilled Licensed Practical Nurse (LPN). Join Champlain Valley Senior Community, a dynamic and caring senior living facility nestled in the beautiful Adirondack Mountains.
Responsibilities
- Provide nursing care within LPN scope aligned with individualized resident service plans
- Monitor and report changes in residents’ physical, emotional, cognitive, or behavioral conditions promptly
- Administer medications and treatments as ordered and maintain accurate documentation
- Communicate effectively with healthcare providers, families, and care team members
- Assist with admissions, evaluations, and care plan updates
- Respond calmly and efficiently to resident emergencies and incidents
- Support infection control, medication management, and safety protocols
- Lead and guide care team members including HHAs and medication technicians
- On call schedule when needed for aftr hiuorse hospice or nedication coverage
Qualifications
- Current and unrestricted New York State LPN license
- CPR certification or willingness to obtain promptly
- Experience preferred in assisted living, memory care, or geriatrics
- Strong clinical, communication, and leadership skills
- Comfortable with electronic health records and medication systems
- Ability to perform physical duties safely and respond well in emergencies
