Pay Ranges: $60.00 - $70.00 per hour (Based on Experience)
Bonus for Full-Time Position: $4,000 Hiring & Retention Bonus (Limited Time Offer)

This role focuses on improving mobility, strength, balance, and functional independence to enhance residents’ safety and overall quality of life. Reporting: This position collaborates closely with the rehabilitation team, interdisciplinary staff, and physicians to support comprehensive care planning.
Role Overview
- Evaluate residents and develop individualized physical therapy treatment plans
- Implement evidence-based interventions to improve functional outcomes
- Document resident progress accurately and in compliance with regulatory requirements
- Collaborate with interdisciplinary teams and physicians
- Supervise Physical Therapist Assistants and therapy aides, as applicable
- Monitor clinical outcomes and contribute to departmental performance
What We’re Looking For
- Active California Physical Therapist license in good standing
- Bachelor’s, Master’s, or Doctorate degree in Physical Therapy
- New graduates are welcome to apply; experience in skilled nursing or rehabilitation settings preferred
- Knowledge of PDPM, Medicare, and skilled nursing facility regulations
- Strong leadership, communication, and organizational skills
- Commitment to resident-centered, high-quality care
