RN Case Manager
Position Summary
The RN Case Manager is responsible for coordinating and overseeing the delivery of rehabilitation and medical services for residents in a Skilled Nursing Facility. This role collaborates closely with the interdisciplinary team to ensure cost-effective, high-quality care while facilitating admissions, ongoing utilization review, and discharge planning. The RN Case Manager serves as a resident advocate and primary liaison between the facility, insurance payers, and external medical management professionals.
Qualifications
- Current California RN license in good standing
- Minimum of 3 years of clinical experience in a hospital, skilled nursing facility, or related healthcare setting
- Strong working knowledge of managed care, Medicare, and utilization review guidelines
- Excellent verbal and written communication skills
- Proven ability to multitask, prioritize, and meet deadlines
- Ability to work both independently and collaboratively while following established procedures and regulations
- Demonstrates professionalism, accountability, and solution-oriented thinking
- Displays emotional maturity with the ability to remain calm, focused, and professional in high-pressure situations
- Treats residents, families, and coworkers with respect, kindness, and professionalism at all times
Responsibilities
- Welcome and orient new residents, ensuring a positive and supportive admission experience
- Assess, monitor, and communicate resident clinical status, functional changes, and care plan updates to insurance payers
- Document all payer communications, including resident progress, anticipated outcomes, authorizations, and special instructions
- Participate in and collaborate with the interdisciplinary team to coordinate services and maintain continuity of care
- Coordinate with the Business Office and Medical Records departments to ensure accurate census reporting and timely reimbursement for managed care and Medicare residents
- Support efficient discharge planning to promote safe transitions of care
