What You'll Do
Responsibilities may vary based on each client's needs, the individualized plan of care, the position, and the applicable scope of practice and may include:
- Completing nursing assessments and identifying changes in a client's condition
- Developing, reviewing, or updating individualized plans of care, as assigned and permitted
- Providing skilled nursing care, treatments, and procedures within the applicable scope of practice
- Monitoring client outcomes and promptly communicating significant changes or concerns
- Maintaining accurate, detailed, and timely clinical documentation
- Coordinating with providers, the clinical team, clients, and family members as appropriate
- Providing client and family education related to the plan of care
- Providing clinical supervision or oversight when included in the position's responsibilities
- Following infection-prevention, safety, and emergency procedures
- Maintaining client dignity, privacy, and confidentiality
What We're Looking For
Required Qualifications:
- Current unrestricted Wisconsin Registered Nurse (RN) license
- Graduate of an accredited school of nursing
- Minimum one year of clinical nursing experience
- Valid driver's license, reliable transportation, and automobile insurance
- Strong communication, organizational, and clinical assessment skills
- Ability to work independently, exercise sound clinical judgment, and collaborate effectively with a multidisciplinary team
- Proficiency with computers and electronic documentation systems
Preferred Qualifications:
- Home care, home health, hospice, or community-based nursing experience
- Experience performing client assessments and care planning
- Experience providing patient and family education
