What You'll Do
- Provide direct patient home care visits, observes, performs and reports preventative, treatment and rehabilitative nursing procedures while implementing the plan of care
- Observe and report on the home environment and make recommendation that will enhance patient care, promote safety and prevent adverse effects
- Work with patient, family and health care team members to coordinate services, promote quality of care and positive patient care outcomes
- Take verbal orders from Health Care Providers as necessary and within the LVN scope of practice in a timely manner and documents appropriately
- Inform Health Care Providers of changes in patient’s condition in accordance with providers orders and documents communications accordingly
- Provide care as ordered that is appropriate preventive, rehabilitative, therapeutic, comfort and pain interventions
- Provide instructions to patient/CG’s and staff in a timely manner
Who You Are
- A compassionate and dependable Licensed Vocational Nurse (LVN) with experience or interest in home health nursing
- Comfortable providing skilled nursing care in the home setting while working independently
- Skilled at building meaningful relationships with patients and caregivers
- Committed to delivering high-quality care to patients recovering from illness, surgery, or injury, as well as those managing chronic conditions
- Motivated by improving patient outcomes, quality of life, and reducing avoidable hospitalizations
- Organized, communicative, and confident in patient education and documentation
Qualifications
- Graduate of an accredited Licensed Vocational Nurse (LVN) nursing program
- Current, active California LVN license and CPR certification
- Minimum of one (1) year clinical nursing experience; home health experience preferred
- Ability to provide skilled nursing care in the home, including assessments and patient education
- Valid driver’s license, reliable vehicle, and current auto insurance (required for home visits)
- Strong communication, documentation, and organizational skills
- Experience with home health documentation systems preferred (HCHB a plus)
