What You'll Do
- Report changes in patient condition and safety concerns promptly
- Deliver PTA services per plan of care (therapeutic exercise, gait, transfers, functional training)
- Use physical agents as directed (heat, cold, ultrasound, etc.)
- Observe, document, and report patient progress and response to treatment
- Communicate effectively with PT, Clinical Manager, and care team
- Comply with State, Federal, Medicare, and payer requirements
- Complete accurate, timely clinical documentation
Who We’re Looking For
- Patient-focused and passionate about rehabilitation
- Comfortable working independently in home health
- Reliable, organized, and accountable
- Strong communicator with patients and caregivers
- Team-oriented and responsive to clinical leadership
Qualifications
- Active Physical Therapist Assistant (PTA) license in the state of California
- Minimum 1 year of PTA experience under PT supervision
- Valid driver’s license, reliable transportation, and auto insurance
- Ability to travel to patient homes
- Strong documentation skills
- Home health and Home Care Home Base experience preferred
