What You'll Do

  • Report changes in patient condition and safety concerns promptly
  • Deliver PTA services per plan of care (therapeutic exercise, gait, transfers, functional training)
  • Use physical agents as directed (heat, cold, ultrasound, etc.)
  • Observe, document, and report patient progress and response to treatment
  • Communicate effectively with PT, Clinical Manager, and care team
  • Comply with State, Federal, Medicare, and payer requirements
  • Complete accurate, timely clinical documentation

Who We’re Looking For

  • Patient-focused and passionate about rehabilitation
  • Comfortable working independently in home health
  • Reliable, organized, and accountable
  • Strong communicator with patients and caregivers
  • Team-oriented and responsive to clinical leadership

Qualifications

  • Active Physical Therapist Assistant (PTA) license in the state of California
  • Minimum 1 year of PTA experience under PT supervision
  • Valid driver’s license, reliable transportation, and auto insurance
  • Ability to travel to patient homes
  • Strong documentation skills
  • Home health and Home Care Home Base experience preferred
