With our continued growth, we're expanding our clinical coverage to better serve patients across the region. We have two full-time opportunities available, with one position focused on North Riverside County and the other on South Riverside County. Whether you're located in the northern or southern part of Riverside County, we'd love to connect and explore which territory best aligns with your location and interests!

What You'll Do
As a Home Health Physical Therapist, you'll provide individualized, skilled therapy services to patients in the comfort of their homes, helping them achieve meaningful rehabilitation goals. Key responsibilities include:
- Conduct comprehensive physical therapy evaluations and develop individualized treatment plans.
- Provide skilled therapeutic interventions to improve mobility, strength, balance, and functional independence.
- Assess patient progress, update treatment plans, and communicate changes to physicians and the clinical team.
- Complete timely and accurate clinical documentation, including OASIS assessments when applicable.
- Collaborate with nurses, therapists, physicians, and other interdisciplinary team members to coordinate patient care.
- Educate patients and caregivers on exercises, safety, fall prevention, and strategies to promote independence.
- Provide guidance, supervision, and consultation to Physical Therapist Assistants as appropriate.
- Support positive patient outcomes and help reduce avoidable hospitalizations.
What We're Looking For
- Graduate of an accredited Physical Therapy program.
- Current, active California Physical Therapist license in good standing.
- Minimum of one year of professional Physical Therapist experience preferred.
- Home health experience preferred; clinicians with strong rehabilitation backgrounds are encouraged to apply.
- Familiarity with Homecare Homebase (HCHB) and OASIS documentation is a plus.
- Strong clinical assessment, communication, and organizational skills.
- Valid driver's license, reliable transportation, and current automobile insurance.
- Ability to independently manage patient visits and complete clinical documentation in a timely manner.
