Physical Therapist (PT)

Job Summary


A registered professional who provides coordinated care to patients of all age groups. Plans, organizes, and conducts physical therapy treatment programs based on the medical referral and their evaluation. Collaborates with interdisciplinary team. Successful candidates will receive a $5,000 sign-on bonus after the first 90 days of employment.


Duties and Responsibilities:

  • Perform initial and ongoing clinical assessments to identify patient needs, and determine level of functioning, including OASIS assessments at appropriate time points and appropriateness of patient for home health
  • Develop and revise a Plan of Care in consultation with the physician and other care team members
  • Provide therapeutic treatments and evaluation of equipment needs as applicable
  • Evaluate outcomes of care with appropriate discharge planning
  • Apply concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease
  • Accept clinical assignments that are consistent with education and competence to care for patients
  • Supervise PTA and HHA as appropriate
  • Meet mandatory continuing education requirements of the agency/licensing board and participate in in-service programs
  • Wound care

Required Skills/Abilities:

  • Therapy skills as defined as generally accepted standards of practice
  • Good interpersonal skills
  • If required to make visits need proof of current CPR, TB screening, and Hepatitis profile
  • Knowledge of durable medical equipment
  • Ability to work flexible schedule
  • Ability to travel locally
  • Transportation: Reliable transportation and valid and current auto liability insurance

Required Education and Experience:

  • Bachelor of Science in Physical Therapy from an American Physical Therapy Association-approved program
  • Current Texas State License as a Physical Therapist
  • Current Texas Drivers License
  • Two years' experience as a Physical Therapist in an acute care or rehabilitation setting

Supervision Responsibilities:

  • This position has supervisory responsibilities within PTA and HHA as appropriate

Physical Requirements

  • Occasionally required to sit
  • Frequently required to talk and stand
  • Frequently required to utilize hand and finger dexterity
  • The employee must occasionally lift and/or move up to 50 pounds
  • Specific frequently vision abilities required by this job include: Close vision, Color vision, Peripheral vision, Depth perception, and Ability to adjust focus

Work Environment:

  • BLOODBORNE PATHOGENS EXPOSURE LEVEL: 1 (HIGH)
  • Noise Level: Moderate
  • Requires working under some stressful conditions to meet deadlines and patient needs and to meet patient/family individualized psychosocial needs
