As a Population Health Medical Assistant, you’ll play a vital role in advancing preventive care, quality performance, and patient experience—while growing your career in value‑based healthcare.
Your Responsibilities:
Inbound Clinical Calls & Patient Support
- Manage inbound clinical calls related to Annual Wellness Visits (AWVs), screenings, immunizations, and follow-up care
- Provide clear guidance to patients and escalate issues appropriately
- Deliver compassionate, professional patient support every interaction
Annual Wellness Visit (AWV) Quality Support
- Perform pre-screens for all scheduled AWVs to identify quality opportunities
- Complete detailed chart reviews post-AWV to ensure documentation accuracy
- Identify care gaps and initiate appropriate follow-up actions
Quality Measures & Care Gap Focus
- Review and address key quality measures, including:
- Mammogram screenings
- Colorectal cancer screenings
- Immunization status
- Oral health screenings
- Other assigned quality initiatives
- Support workflows to ensure accurate quality reporting and performance results
Patient Outreach & Experience
- Conduct post-AWV follow-up calls to:
- Gather patient experience feedback
- Reinforce outstanding care gaps
- Confirm understanding of next steps
- Educate patients on collecting and returning colorectal cancer self-collection kits
Reporting & Administrative Support
- Document patient interactions, chart reviews, and outreach activities in the EHR
- Track quality gaps and outcomes using Excel or reporting tools
- Participate in virtual weekend rotation coverage to support population health operations
What You’ll Need:
Required Qualifications
- High School Diploma or GED
- Certified Medical Assistant (CMA) – REQUIRED
- Minimum 3 years of clinical experience with an operational focus on quality or population health
Preferred Qualifications
- LVN or degree in Healthcare Management or related field
- Experience with Value‑Based Care, quality metrics, or ACO environments
- Hands-on experience supporting performance improvement initiatives
Skills & Abilities
- Strong phone presence and patient communication skills
- Experience with chart review and quality metric identification
- Highly organized with excellent attention to detail
- Comfortable in a task-driven, metric-focused environment
- Basic to intermediate Excel and reporting skills
- Ability to communicate effectively with groups of 10+ people
- Proven ability to track daily objectives and present results to leadership
- Independent, self-driven, and team-oriented
