Responsibilities for the Part-time RN/Social Worker:

• Collaborate with the rest of the team to supervise our care providers – Caregivers, LVN’s and RN’s.
• Completing intake information on new clients and entering Care Plan into our care management system.
• Provides Nurses with clinical information, supported by evidence-based practice, to create and direct health care providers in providing the best care for individual clients.
• Supervises all Nurses and reviews all newly admitted, current, and re-certified client charts, to ensure proper implementation of care. Formulating unique Plans of Treatment based on the needs of each client, according to MD order and RN evaluation, and establishing care coordination as necessary for all clients.
• Makes recommendations regarding the hiring, advancement, retention, or termination of all employees supervised.
• Reviews field staff flow sheets/notes; performs assessments on clients as needed; communicates effectively with other health care professionals, families and clients.

Qualifications:

• Current California Registered Nurse License or Social Worker License
• Required Minimum of 3 years nursing experience, preferred in the Hospice and/or Home Health industry. Experience in home care preferred
• Able to demonstrate clinical nursing expertise and management/leadership through skills evaluation
• Minimum of 1-year supervisory experience in Hospice, hospital, or Home Health (Leadership position preferred)
• Valid CPR, Drivers License, auto insurance
• Able to effectively communicate in English, additional language a PLUS
