Job Purpose or Summary:
In alignment with company mission, vision and values, the Community Liaison serves as the outward-facing
representative of Arosa, responsible for developing, nurturing, and maintaining strong relationships with referral
sources, community partners, and prospective clients.This role plays a key part in increasing brand awareness,
growing census, and ensuring that aging adults and their families are aware of and connected to the care and
support they need.
Essential Duties & Responsibilities:
Referral Development & Community Outreach
● Build and maintain strong relationships with hospitals, physician offices, skilled nursing facilities, senior
living communities, elder law attorneys, care managers, and other community-based referral sources.
● Identify, develop, and execute outreach strategies to generate qualified leads and inquiries for home
care and care management services.
● Represent the company at networking events, health fairs, and professional associations to enhance
community presence and visibility.
● Provide education to community partners and prospective clients on the company’s services, philosophy
of care, and differentiators.
Sales Process & Lead Conversion
● Follow up with all inquiries, leads, and referrals in a timely and compassionate manner.
● Collaborate with intake and care coordination teams to ensure a seamless client onboarding
experience.
● Track outreach activities, lead sources, and conversion data using CRM tools (e.g., Salesforce).
● Meet or exceed established growth targets and conversion benchmarks.
Market Intelligence & Strategy
● Monitor local market trends, competitor activities, and community needs.
Provide feedback and insights to leadership on opportunities for service expansion, messaging, or
process improvements.
● Support marketing campaigns and promotions tailored to specific referral partners or market segments.

Knowledge and Skill Requirements:
● 2+ years of experience in community outreach, business development, sales or case management -
preferably in home care, healthcare, or senior services.
● Proven ability to build and maintain trusted relationships with professionals and families.
● Strong communication, presentation, and interpersonal skills.
● Highly organized, self-motivated, and goal-oriented.
● Knowledge of the local healthcare landscape, senior care services, and referral dynamics preferred.
● Valid driver’s license, proof of current auto insurance, and reliable transportation required.
