This role will assist in the growth of our business and deliver high-quality patient care.
Build and maintain relationships with local providers and referral sources
Lead community outreach to increase client census
Provide clinical leadership for the care team
Conduct initial assessments and weekly care management
Triage clinical situations
Coordinate care with the Interdisciplinary Team (IDT) and create care plans for caregivers
Ideal candidates bring strong nursing experience who are comfortable with community outreach.
The RN Care Manager Duties include but are not limited to the following:
Provide clinical support to in-home caregivers
Deliver nursing education to patients, family and staff as needed
This hybrid position requires the RN Care Manager to be confident in providing basic
bedside nursing care
Intake Assessments and Consultations
Community Outreach/Support
Medication management
Care Coordination
Patient Advocacy
Transport clients to appointments and outings
Knowledge and Skill Requirements:
- Educational Minimum Requirement: an Associate Degree in Nursing; Preferred
- candidate: Bachelor’s or Master’s Degree in Nursing.
- Preferred Med/Surg, Emergency or Intensive Care Experience
- Must be a Registered Nurse (RN) in the state for which they are applying
- A minimum of two (2) years’ experience in geriatrics, home care, home health, and/or
- care management required; other professional experience may substitute.
- Ability to work in a team environment
- Ability to interface effectively and professionally with clients family, physicians, attorneys
- and other professionals included in client’s case
- Excellent written and verbal communication skills required.
- Technical literacy required
- Valid driver’s license and proof of current auto insurance required
