JOB DESCRIPTION: Case Manager
DEPARTMENT: New Beginnings
REPORTS TO: Director of CE and CQI
FLSA STATUS: SALARY– Non – Exempt
CLASSIFICATION: Full Time (40 HRS)
SHIFT: Monday – Friday (require some evenings and weekends)
WORK LOCATION: On-site
COMPENSATION: $45,000/annually Employee benefit package eligible for full time
PROBATION PERIOD: 90 – Days
POSITION SUMMARY:
Under the supervision of the Director of CE and CQI, the Case Manager will be responsible for providing diversion assistance to clients in need. The Case Manager will work as a cohesive team to deliver prevention, diversion, and rapid exit services to individuals and families experiencing a housing crisis.
PRINCIPAL ACCOUNTABILITIES:
- Provide real-time, problem-solving solutions to individuals in the New Beginnings Program
- Provide case management services, including but not limited to intake, assessment, screening, conflict mediation and negotiation, crisis housing search, short-term diversion case management assistance, financial assistance, legal assistance, and linkages to community resources and mainstream benefits
- Determine type and amount of eligible financial supports and submit check requests for approved financial assistance payments
- Ensure all assessments are administered and services and case notes are entered into the HMIS database within 24 hours of contact
- Offer flexible office hours at multiple locations within the community. Locations may include, but not limited to, shelter locations, encampments, courts, housing properties, partnering agencies and other community based organizations. Work hours may include nights and weekends to accommodate the needs of clients.
- Remain informed on CoC and HMIS database updates
- Attend Partners for Home and City of Atlanta CoC provider team meetings as well as case conferencing meetings
- Build internal and external relationships with partners that will support clients in their ability to reach their goals
- Work in collaboration with partner agencies toward the full implementation of centralized assessment, referral, and placement
- Be knowledgeable about CoC guidelines and procedures in order to answer questions posed by outside agencies
- Participate in staff development and all required agency meetings
- All other duties as assigned
QUALIFICATIONS:
- Bachelor’s degree in social services or related field required with a minimum of two (2) years of case management experience or social services experience
- Valid Georgia Driver’s License
- Strong organizational skills
- Excellent written and verbal communication skills
- Proficient in Outlook and Microsoft Word and other software packages
- Flexibility (Position will require some evenings and weekend)
