JOB DESCRIPTION: Case Manager – Veterans Program
Reports To: VA PROGRAM SUPERVISOR
Position Status: Full Time (40 HRS)
POSITION SUMMARY:
The Case Manager in the Gateway Center’s Veterans Program is responsible for providing client centered case management services and evidence-based intervention strategies. Case management services support program participants with their desire to end their
current condition of homelessness; maintain a linkage to care for medical and behavioral health; identify employment or employment training opportunities; and to become stably housed. This position will facilitate and support the provision of structured group activities including discussions on recovery and physical, social and recreational activities for men residing in Veteran’s program. This position along with two (2) additional case managers, will support and provide case management services to fifty-six (56) men that reside in the Veterans Program at the Gateway Center (46 Long Term – 2 Year Beds and 10 Short Term – 60 Day Beds). This position will utilize motivational
interviewing: a coaching model for the approach to case management that supports moving clients towards becoming self-sufficient.
The overall goals of the position are to improve the client’s ability to remain linked to medical and behavioral healthcare; support and coach client to access income through employment or other benefits; family reunification and to identify housing options that support the client in
being stably housed at discharge.
PRINCIPAL ACCOUNTABILITIES:
Encourage and support client participation in activities and events that increase ability to
remain linked to care and/or behavioral health care, identify employment options or cash benefits, and identify housing options.
Build internal and external relationships with partners that will support clients in their ability to reach their goals
Develop goals through the use of an Individual Service Plan (ISP) and monitor progress
Support and monitor individual progress through case management notes
Provide one-on-one case management sessions for identified clients weekly or as needed based on needs and service plan
Encourage and support client participation in activities and events that increase social capital in community
Ensure all case records are current by documenting ongoing contacts through case notes within 72 hours, trainings attended, referrals, and resources offered
Maintain complete and current documentation on all clients in required databases
Submit monthly reports to supervisor
Participate in staff development and all required Agency meetings
Other duties as assigned
QUALIFICATIONS:
Bachelor’s degree in social service field preferred (i.e. social work, counseling, child and family services, human services)
High school diploma with a minimum of two (2) years of case management experience or social
services experience will be considered.
Strong organizational skills and commitment to substance abuse and disease concept focused treatment
models.
Excellent written and verbal communication skills
Proficient in Outlook and Microsoft Word and other Software Packages
Flexibility (may require some evenings and weekend work)
