This is a split role providing speech therapy coverage across both facilities, not necessarily on the same days, offering clinical variety and schedule flexibility that a single-site position simply cannot match.

The Role
The Speech Language Pathologist is a licensed healthcare professional responsible for the comprehensive evaluation, diagnosis, treatment planning, and implementation of speech, language, cognitive-communication, and swallowing disorder management programs across Oakland Heights Nursing and Rehabilitation and Lake Park Healthcare Center. This is a split Part-Time role with coverage at both sites as scheduled. This position is classified as Non-Exempt and reports to the Director of Rehabilitation and the Facility Administrator at each site.
Core responsibilities include:
- Conducting comprehensive speech-language and swallowing evaluations in accordance with physician orders and California standards of practice
- Assessing and diagnosing speech, language, voice, fluency, cognitive-communication, and dysphagia disorders
- Developing, implementing, and modifying individualized plans of care with measurable, function-based goals consistent with benefit and reimbursement levels
- Providing skilled therapeutic interventions addressing communication, cognition, swallowing safety, and compensatory strategy development
- Monitoring patient progress and adjusting treatment plans based on objective findings and clinical response
- Supporting and instructing patients and families in techniques and behavioral adaptations to promote better communication and swallowing outcomes
- Leading discharge planning and providing recommendations to support safe transitions of care and community reintegration
- Communicating evaluation findings, progress updates, and discharge outcomes to physicians, interdisciplinary team members, patients, and families while maintaining confidentiality in accordance with HIPAA
- Educating staff, other healthcare professionals, and third-party payers on the benefits and functional outcomes of speech therapy interventions
- Participating in care conferences, interdepartmental meetings, quality assurance initiatives, and collaborative planning at both facilities
- Maintaining compliance with HIPAA, patient rights, OSHA, infection control, and all applicable California state practice standards
- Treating all residents with dignity, respect, and compassionate professionalism
Physical requirements include the ability to assist with patient positioning during evaluations and treatment, performing oral mechanism examinations and swallowing assessments, prolonged sitting, standing, walking, bending, reaching, and computer-based documentation. The ability to assist in resident evacuation during emergency situations is also required.
What You Bring
- Current, active, and unrestricted Speech Language Pathologist license in the State of California — required (pending licensure considered)
- Master's or Doctoral degree in Speech-Language Pathology from an accredited program — required
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA — required if mandated by facility or payer guidelines
- Current CPR/BLS certification (or willingness to obtain per facility policy)
- Ongoing compliance with continuing education requirements established by the California Speech-Language Pathology and Audiology and Hearing Aid Dispensers Board
- Experience in a skilled nursing, post-acute, hospital, or related medical setting — strongly preferred
- Excellent verbal and written communication skills with the ability to exercise sound clinical judgment and lead by example
- Composure and professionalism in dynamic, fast-paced clinical environments
- Strong organizational skills and the capacity to manage responsibilities across two facility environments
